Abstract. This panel will address the research challenges of implementing large-scale health information systems in the digital health research domain. Although this can be a move from the “pilotitis” situation recognized by implementing small-scale pilots that fail to scale up, it raises other challenges. Some of these challenges, such as fragmentation, limited stakeholder involvement, and lack of learning and direction, can be covered by the self-developed new term "pilotanic". Pilotanic is introduced to encapsulate the phenomenon where ambitious health information initiatives, intended to overcome pilotitis and make large steps to improve healthcare delivery, encounter significant barriers or fail outright. Drawing upon insights from practitioners, researchers, and policymakers, this panel aims to dissect the root causes of failure of large-scale health information systems and explore strategies for overcoming it. By shifting the focus from small-scale pilots to sustainable, scalable solutions, panelists will engage in lively discussions and collaborative problem-solving to avoid pilotanic and advance the field of digital health research.
